Giving Home Health Care in Albuquerque, NM is seeking a dedicated Patient Coordinator to join our care team. You will act as a liaison among patients, caregivers, clinical staff, and internal departments, ensuring timely communication, coordinated care, and a positive patient experience.

The role involves home visits, caseload management of 50+ patients, documentation in Salesforce/Igea, training for staff, and working with onsite nurses under the Care Team Director.

Qualifications

• 2–5 years of customer service and/or case management experience.
• Strong written and verbal communication skills.
• Proficient in MS Office Suite and comfortable using tablets/laptops.
• Understanding of HIPAA regulations and patient confidentiality.
• Valid New Mexico driver’s license and reliable vehicle for patient visits.
• Flexibility to work outside normal hours as patient needs require.

Responsibilities

• Serve as the primary liaison between patients, caregivers, clinical staff, and internal departments, ensuring clear and timely communication throughout the care continuum.
• Conduct home visits to build and strengthen patient relationships, initiate homecare services, and gather necessary compliance documentation.
• Attend and advocate for patients at physician appointments, coordinating home care and DME arrangements.
• Manage a caseload of 50+ patients, coordinating with families and healthcare providers for documentation and renewals.
• Ensure patients have proper DME and provide training to patients, aides, and nurses.
• Update and upload documentation in Salesforce and EMR within 24 hours of visits.
