Qualifications

• High school diploma or GED required.
• Relevant experience in the field of patient transportation or care coordination.
• Licensure may be required depending on position requirements.

Responsibilities

• Coordinate patient discharge transportation using ambulances, wheelchair vans, taxis, buses, and other resources.
• Communicate with patients, families, clinical teams, case managers, and transport vendors to arrange transport.
• Navigate insurance coverage requirements and transportation eligibility guidelines for discharge planning.
• Prioritize transportation requests while managing multiple cases in a dynamic healthcare setting.
• Investigate transportation concerns, resolve service issues, and support positive patient experiences.
• Track transportation activity, process invoices, and maintain accurate records according to procedures.
• Assist with process improvement initiatives and onboarding of new team members.
