Qualifications

• High school diploma or GED required.
• Relevant experience in transportation coordination or healthcare support preferred.
• Excellent communication and organizational skills.
• Ability to manage multiple priorities in a fast-paced environment.

Responsibilities

• Coordinate patient discharge transportation using ambulances, wheelchair vans, taxis, buses and other resources.
• Communicate with patients, families, clinical teams, case managers and vendors to arrange transport solutions.
• Navigate insurance coverage requirements and transport eligibility guidelines for discharge planning.
• Prioritize transportation requests while managing multiple cases in a dynamic healthcare setting.
• Investigate transportation concerns, resolve service issues and support positive patient experiences.
• Track transportation activity, process invoices, and maintain accurate records.
• Support process improvement initiatives and assist with training and onboarding.
