Job summary

UVA Health in Charlottesville is seeking a Discharge Transportation Coordinator to serve as the central link among care teams, transportation providers, patients, and families, ensuring safe, timely transitions from hospital to the next stage of care.

You will coordinate ambulances, wheelchair vans, taxis, and other options from UVA Health Town Center One, balancing patient needs with insurance requirements and operations to move patients efficiently.

Qualifications

• Education: High school diploma/GED.
• Experience: Relevant experience in the field.
• Licensure: May be required based on position requirements.

Responsibilities

• Coordinating patient discharge transportation using ambulances, wheelchair vans, taxis, buses, and other transportation resources.
• Communicating with patients, families, clinical teams, case managers, and transportation vendors to arrange appropriate transport solutions.
• Navigating insurance coverage requirements and transportation eligibility guidelines to support efficient discharge planning.
• Prioritizing transportation requests while managing multiple cases simultaneously in a dynamic healthcare environment.
• Investigating transportation concerns, resolving service issues, and supporting positive patient experiences.
• Tracking transportation activity, processing invoices, and maintaining accurate records in accordance with departmental procedures.
• Supporting process improvement initiatives and assisting with training and onboarding of new team members.
