Job summary

Naples Comprehensive Health - NCH is seeking a Hospital Discharge Planning Assistant to coordinate patient transitions from hospital to the next level of care. You will work with the healthcare team, patients, and families to ensure discharge plans are effective and care resources are arranged.

This patient-facing role emphasizes enhancing patient experience, collaborating with RN and social work teams, and ensuring timely CMS notifications and accurate documentation in EPIC.

Qualifications

• High School/GED required; hospital experience preferred.
• Proficient with Microsoft Office.
• Strong communication skills with diverse populations.
• Highly detail oriented.

Responsibilities

• Assists RN and SW Case Managers in discharge coordination and clerical functions.
• Assists with referral processes by collecting and sharing information to support provider and service selection.
• Coordinate post-discharge placement, verify insurance, and obtain prior authorizations as needed.
• Communicate with community resources to support discharge transitions.
• Document interventions in EPIC and maintain resource lists.
