Job summary

Advocate Health is seeking an experienced Registered Nurse to lead discharge planning and care coordination in the Social Services: ACH department. You will assess patient needs, develop care plans, and coordinate services to ensure safe transitions from hospital to next care setting.

Responsibilities include collaboration with patients, families, and the care team, utilization review, regulatory compliance, and timely documentation of discharge planning activities.

Qualifications

• Ability to prioritize and organize work; effective communication and critical thinking.
• Navigate the Electronic Health Record and basic MS Office use; knowledge of Medicare A and B.
• Knowledge of Managed Care requirements and Utilization Management; discharge planning experience.

Responsibilities

• Conducts assessments, develops plans, and initiates timely interventions for discharge planning.
• Collaborates with patient, family, and team to align care with goals and ensure smooth transitions.
• Initiates referrals and documents discharge planning activities per standards; uses CMS guidelines.
• Maintains regulatory and utilization management documentation and coordinates with care teams.
