PAM Health is opening a brand-new rehabilitation hospital in Aiken, SC, with a focus on high-quality patient care. The Patient Assessment Coordinator will drive accurate data collection for Medicare reporting, support CMS compliance, and guide multidisciplinary teams in IRF/PAI processes.

Qualifications

• Graduate of an accredited OT, PT, SLP, or RN program with current licensure.
• ASN required; Bachelor’s degree preferred for nursing.
• Master’s degree required for OT/PT/SLP.
• Current BLS/CPR card.

Responsibilities

• Coordinate data collection for IRF/PAI forms.
• Ensure documentation supports CMS guidelines.
• Monitor accuracy of FIM scoring and patient care needs.
• Set IRF/PAI completion dates and educate staff.
