Job summary

PAM Health is seeking a Patient Assessment Coordinator to coordinate data collection for the IRF/PAI under CMS guidelines. You will ensure timely submission, accuracy of functional status scoring, and staff education on policy and procedure for Medicare reporting.

The role requires licensure in the state and BLS/CPR certification, with 3–5 years of related experience in an acute hospital, rehabilitation, or LTACH setting. This position offers a competitive hourly pay and growth opportunities.

Qualifications

• Three to five years of experience in a patient assessment coordinator role.
• Graduate of an accredited OT, PT, SLP, or RN program with current licensure.
• Current Basic Life Support (BLS)/CPR certification.

Responsibilities

• Coordinate data collection for IRF/PAI under CMS guidance.
• Review IRF/PAI forms for accuracy and timeliness.
• Educate staff to ensure CMS compliance and proper FIM scoring.
