As a Patient Coordinator, you'll serve as the primary liaison between our patients and providers, ensuring every patient feels supported throughout their healthcare journey. You'll build meaningful relationships, guide patients through their personalized care plans, coordinate follow-up care, and proactively reconnect patients who have fallen out of care. By combining exceptional service with strong organizational skills, you'll help create a seamless patient experience while optimizing provider schedules and ensuring no patient falls through the cracks.

About the Role

As our Patient Coordinator, you'll serve as the primary liaison between our providers and our patients—guiding individuals through every stage of their healthcare journey.

You'll help transform personalized treatment plans into exceptional patient experiences by ensuring patients understand their next steps, stay engaged in their care, and always know they have someone in their corner.

You'll build trusted relationships, proactively reconnect patients who have fallen out of care, coordinate follow-up plans, educate patients on available services, and remove barriers that prevent them from achieving their health goals.

You'll work closely with our providers, nurses, and leadership team to create a seamless, experience while helping optimize provider schedules and ensuring no patient falls through the cracks.

What You'll Do
Become a Trusted Guide for Our Patients
  • Serve as the primary point of contact throughout each patient's care journey
  • Answer questions, provide education, and coordinate next steps within clinic guidelines
Help Patients Stay Engaged in Their Health
  • Proactively reconnect patients who are overdue for follow-up appointments, laboratory testing, or annual visits
  • Remove barriers that may prevent patients from continuing care
  • Maintain accurate documentation of patient communication and follow-up activities within the EHR
Optimize the Patient Journey
  • Coordinate follow-up appointments and provider recommendations
  • Help optimize provider schedules by filling appointment openings and minimizing gaps
  • Collaborate with clinical staff to ensure patients move efficiently through their treatment plans
  • Guide patients through memberships, IV therapies, supplements, and other services that support their care plan
Deliver an Exceptional Experience
  • Welcome patients with warmth, professionalism, and genuine hospitality
  • Support front desk operations, including check-in, check-out, phone calls, and payments as needed
  • Resolve patient concerns with empathy and professionalism
  • Help continuously improve the patient experience by identifying opportunities to simplify processes and enhance service

Qualifications
  • 2+ years of experience in healthcare, hospitality, concierge service, customer success, patient coordination, or another relationship-focused role
  • Exceptional verbal and written communication skills
  • Highly organized with strong attention to detail
  • Comfortable learning technology and working within electronic health record (EHR) systems
  • Proficient with Google Workspace or similar software
  • Ability to prioritize multiple responsibilities while maintaining exceptional service
  • Passion for helping others and delivering an outstanding patient experience

Preferred

  • Experience in functional, integrative, concierge, or cash-pay healthcare
  • Experience working with EHR systems
  • Sales, hospitality, or membership experience
