Base pay range

$23.00/hr - $23.00/hr

With a focus on personalized, in‑home care, we are committed to assisting those who have dedicated their careers to these vital roles. As a fast‑growing, leading provider, we proudly serve patients across Arizona, Colorado, Kentucky, Missouri, Nevada, New Mexico, Tennessee, Texas, and Utah.

Responsibilities
  • MAJOR RESPONSIBILITIES
  • 1. Patient Support:
  •  a. Will receive the patients from Patient Development department through a warm handoff process;
  •  b. Attend the patients’ doctor visits to provide support, address concerns, and facilitate communication between patients and healthcare providers;
  •  c. Conduct home visits to assess patients’ needs, deliver resources, and provide additional assistance as required;
  •  d. Conduct an initial set up home visit for patients newly approved for home health care. Provide information on the program, as well as facilitate caregiver onboarding;
  • 2. Resource Delivery:
  •  a. Deliver comprehensive patient binders to patients’ homes containing plan of care and training information;
  • 3. Communication:
  •  a. Serve as the main point of contact for patients, ensuring they have a dedicated resource for all inquiries, concerns, and updates regarding their care;
  •  b. Make courtesy calls to patients to check on their well‑being, address immediate concerns, and maintain open lines of communication between the patient and the Giving Home Health Care clinical team;
  •  c. Conduct milestone calls to track patient progress, update them timely regarding their care, approvals, caregiver status, and any other updates to their care;
  • 4. Advocacy:
  •  a. Act as a patient advocate by collaborating with healthcare providers to address patient concerns and ensure patients receive high‑quality, personalized care;
  •  b. Advocate for patients’ needs and rights within the organization, promoting a patient‑centered approach to healthcare delivery
Minimum Qualifications
  • Strong work ethic and relationship building skills
  • Self‑motivated with the ability to manage time appropriately
  • Strong communication skills inside of the office and outside
  • Experience with MS Office products (Outlook, Word, Excel, etc.)
  • Minimum associate level education preferred. (High school diploma or equivalent is required.)
  • Minimum 1‑3 years of experience in a Care Team or Patient Navigation role
  • Must have valid driver's license and your own transportation
  • Dependable transportation to be able to travel locally between locations including patient homes, physician offices and Giving offices
  • Healthcare experience preferred
