Job summary

Giving Home Health Care, LLC in Westminster, CO is seeking a dedicated Patient Coordinator to enhance the patient experience by coordinating care, scheduling visits, and communicating with patients and care teams.

In this full-time role, you will accompany patients on benefit renewal appointments, visit homes to update care plans, ensure HIPAA compliance, and maintain a warm, professional demeanor. Regular hours are 8a–5p with strong benefits after 90 days.

Qualifications

  • 1–3 years in a Care Team or Patient Navigation role preferred.
  • Healthcare experience preferred.
  • Proficient in MS Office; strong empathy and communication.

Since 2012, Giving Home Health Care has been supporting individuals impacted by health conditions related to their work in nuclear facilities for the Department of Energy. We are now hiring for the Patient Coordinator role in our Westminster, CO location!

Responsibilities
  • The Patient Coordinator is responsible for enhancing the overall patient experience and ensuring that patients receive the necessary support and resources.
  • Accompany patients to their benefit renewal appointments to advocate for the patient and the program.
  • Visit patient homes and update patient care.
  • Effectively communicate and provide information to patients, caregivers, and physicians.
  • Understand HIPAA and maintain confidentiality.
  • Display a warm and friendly attitude.
Minimum Qualifications
  • 1 to 3 years of experience in a Care Team or Patient Navigation Role preferred
  • Healthcare experience preferred
  • Key competencies include, empathy, problem solving, communication, active listing, and tenacity
  • Self‑motivated with the ability to manage time appropriately.
  • Experience with MS Office products (Outlook, Word, Excel, etc.)
  • Minimum associate level education preferred. (High school diploma or equivalent is required.)
  • 1 year of medical knowledge preferred.
  • Dependable transportation
Benefits and Compensation
  • $26 - $29 an hour
  • $100 monthly cell phone reimbursement, after 90 days
  • Monthly in-office lunch to celebrate birthdays, anniversaries, and other events
