Job summary

Lansdale Hospital, part of Jefferson Health, seeks a professional nurse to provide patient- and family-centered care using the nursing process. You will develop and implement care plans in partnership with patients and designated care partners, respecting cultural beliefs and individual needs.

The role emphasizes nursing assessments, education, and interprofessional collaboration to improve health outcomes and patient safety. Strong communication and ongoing professional growth are essential.

Qualifications

• Specialized Diploma graduate of an accredited school of nursing required.
• Knowledge of patient- and family-centered practice and collaboration with patients and families.
• Cultural awareness and ability to care for diverse patient populations.

Responsibilities

• Interacts with co-workers, patients, families/designated care partners, and staff per Jefferson Health values.
• Integrates evidence-based practice and clinical expertise to provide safe, high-quality care.
• Partners with patients and families to identify learning needs and provide education on conditions, treatment and discharge plans.
• Minimizes risk by following established nursing practices and protocols and by using data to monitor outcomes.
• Communicates using information technology to support care delivery and documentation, and participates in quality improvement activities.
• Active participation in hospital and practice setting committees and ongoing professional development.
