Job summary

ERC Pathlight is seeking a Treatment Coordinator in Colorado to greet patients, coordinate admissions and transitions across levels of care, and provide administrative support to treatment teams. The role focuses on a positive patient experience and timely completion of plans and surveys.

Qualifications

• High school diploma or GED is required.
• Bachelor’s degree in healthcare, business, or related field preferred.

Responsibilities

• Admissions and patient transitions coordination.
• Sends welcome emails or other information to patients/families; coordinates referrals.
• Assists with patient admissions in medical records system and portal.
• Greets and assists visitors; answers phones and directs calls.
• Maintains patient/family binders and discharge packets.
• Coordinates transportation and schedules meeting spaces.
• Supports treatment teams; distributes assessments and surveys.
