Purpose and Scope:
Functions as part of the dialysis health care team in providing safe and effective home dialysis therapy and training for patients under the supervision of a licensed nurse, following organization policies, procedures, and regulations set by the organization, state, and federal agencies. Assists in setting up home dialysis equipment and maintaining a safe, clean environment within the home therapy program. Supports the organization's commitment to the Quality Enhancement Program (QEP) and Continuous Quality Improvement (CQI) activities, including patient satisfaction initiatives, and participates in process improvements to achieve the organization's Quality Enhancement Goals (QEP).
All duties are performed under the supervision of the Home Therapies registered nurse (HT RN), in accordance with applicable laws, regulations, and policies.
Principal Duties and Responsibilities (Not Home Assist):
- Reinforces home hemodialysis (HHD) and/or peritoneal dialysis (PD) training as permitted by law.
- Identifies and welcomes patients, obtains vital signs, weights, and gathers information for the Clinic Visit Intake form.
- Builds communication and rapport with patients, care partners, staff, providers, and team members.
- Obtains and reviews treatment records, reports missing or abnormal data to the HT RN.
- Assists with electronic treatment, medication, and nutritional supplement reconciliation, reporting issues to the HT RN.
- May assist with home visits and provides home support as permissible.
- Prepares supplies for laboratory testing and specimens for shipping.
- Tracks and ships transplant-related laboratory specimens and reinforces transplant education.
- Completes daily chores, disinfects equipment, prepares treatment rooms, stocks supplies, and manages inventory efficiently.
- Schedules appointments, contacts patients/caregivers, supports registration, and assists with connected health use.
- Maintains logs, participates in audits, and assists with patient documentation and clerical duties.
Home Hemodialysis:
- Reinforces HHD training and sets up, tests, disinfects HHD machines.
- Troubleshoots equipment issues, prepares dialysate, evaluates access, initiates and monitors treatment, and documents all findings and interventions.
- Performs safety checks, manages post-treatment vitals, and assists with water and dialysate testing.
Peritoneal Dialysis:
- Reinforces PD training, troubleshoots cycler/connectivity issues, performs phlebotomy, and assists with PD setup and exit site care.
- Tracks PD equipment and performs other assigned duties.
Staff Related:
- Participates in recruitment, clinical orientation, ongoing training, and team collaboration to improve awareness of Home Therapies.
Physical Demands and Working Conditions:
- Includes travel, heavy lifting, prolonged standing, and interaction with infectious materials. Accommodations may be provided.
Education and Licenses:
- High school diploma or G.E.D., valid PCT certification/license, and required state licensure.
Experience and Skills:
- At least two years of patient care experience preferred; completion of dialysis training and CPR certification required for continued employment.
