Job summary

Qualifications

• High School Diploma or equivalent required.
• At least 6 months experience in direct patient care preferred.
• Ability to read and write English to provide basic patient care and communicate instructions; basic math for vitals and weights.

Responsibilities

• Provide for care, comfort, and safety under direct supervision of a nurse.
• Assist patients with activities of daily living (bathing, toileting, oral hygiene, linens).
• Record vital signs, weights, and routine measurements in patient charts; report changes.
• Perform delegated nursing tasks as instructed by the nurse.
• Observe patients and report changes to nursing staff; maintain documentation.
