Job summary

CHS in Fort Wayne, IN, is seeking a Patient Care Technician (PCT) to provide high-quality, patient-centered care under RN/LPN supervision. The PCT assists with activities of daily living, maintains a safe care environment, and helps communicate within the healthcare team.

Responsibilities include taking vital signs, documenting data, assisting with meals and hygiene, aiding mobility, and following infection control and PPE protocols.

Qualifications

• 0-2 years of experience in an acute care setting or currently enrolled in a Nursing program preferred.
• BCLS (Basic Life Support) within 90 days of hire required; CPCT/CNA credentials preferred.

Responsibilities

• Assists nursing staff in delivering care and maintaining a clean, safe environment.
• Collects and records vital signs, height, weight, intake/output, and reports findings to RN/LPN.
• Supports patients with meals, bathing, grooming, and hygiene activities.
• Assists with mobility and positioning using walkers, canes, or wheelchairs.
• Collects urine/stool samples and performs glucose monitoring; documents results.
• Communicates patient information effectively to the care team and adheres to PPE protocols.
• Maintains infection control and safety practices; participates in risk management activities.
• May be required to provide continuous observation and other duties as assigned.
