Qualifications

• High School diploma or GED required.
• Completion of an approved PCT/CNA/MA program or equivalent.
• Eligible for the HMH PCT Apprentice Program if not previously completed.
• Ability to learn quickly, work in a team, and deliver high-quality patient care.

Responsibilities

• Provide patient care based on age-specific criteria across the patient population served.
• Ensure safe and effective outcomes through sound judgement and action.
• Maintain ongoing communication with patients, nursing colleagues, and the care team.
• Take initiative to improve performance and patient experience.
• Collaborate on the plan of care from admission to discharge for safe transitions.
• Participate in Unit/Department Performance Improvement activities.
