Job summary

Virtua Health is seeking a patient support teammate for the Med Surg Tele Heart Failure unit. You will assist with daily living activities, support the healthcare team, and help implement the plan of care under RN/LPN supervision.

Responsibilities include vital signs, phlebotomy and EKGs if certified, and promoting patient education and safety. This role emphasizes collaboration, quality care, and ongoing staff training.

Qualifications

• High School Diploma or GED required.
• Completed a PCT course, RN fundamentals nursing course, CNA program, or 1 year direct patient care experience.

Responsibilities

• Assist with activities of daily living and plan of care under supervision.
• Document patient status and communicate changes to RN/LPN.
• Perform vital signs, point-of-care testing, and other procedures as trained (phlebotomy, EKG if certified).
• Reinforce patient and family education on care needs and safety.
• Participate in Virtua patient satisfaction initiatives and performance improvement activities.
• Assist in orientation of new hires.
