Job summary

Ascension is seeking a Nurse Aide for the Inpatient Rehab unit in Mequon, WI on a PRN rotating schedule. You will deliver compassionate personal care, assist with daily living activities, and help ensure safe patient movement during admissions and discharges.

You will observe patient changes, communicate with the nursing team, and document care activities while maintaining a clean, safe environment in a hospital setting. Relevant licensure is required or substitution allowed where applicable.

Qualifications

• BLS Provider within 30 days of hire date (American Heart Association or American Red Cross accepted).
• Nurse Aide credentialed from the Wisconsin Nurse Aide Registry; CNA credentialed with eligibility to work in a hospital setting.
• Education/experience can be substituted for licensure where applicable.
• High School diploma equivalency OR 1 year of applicable cumulative job specific experience required.

Responsibilities

• Deliver compassionate personal care by assisting patients with daily living activities to enhance comfort and healing.
• Support safe patient movement during admissions, transfers, and discharges.
• Observe and communicate changes in patients' physical, mental, or emotional states to the nursing team.
• Document essential patient observations and care activities to maintain continuity of care.
• Sanitize patient rooms and maintain medical equipment to uphold infection control standards.

• Support safe patient movement by guiding individuals during admissions, transfers, and discharges to ensure a seamless and reassuring care transition.
• Protect patient well-being by promptly observing and communicating changes in physical, mental, or emotional states to the nursing team for rapid care adjustments.
• Maintain precise health records by documenting essential patient observations and care activities to safeguard continuity and quality of care.
• Ensure a safe, clean environment by sanitizing patient rooms and maintaining medical equipment to uphold rigorous infection control and safety standards.

What Minimum Requirements You’ll Need

Licensure / Certification / Registration

• BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
• Nurse Aide credentialed from the Wisconsin Nurse Aide Registry obtained prior to hire date or job transfer date required. Certified Nursing Assistant credentialed from the state of Wisconsin, and in Nurse Aide Registry with eligibility to work in a hospital setting obtained prior to hire date or job transfer date. Additional certs may be required by specific job locations.

Education

• High School diploma equivalency OR 1 year of applicable cumulative job specific experience required.
• Note: Required professional licensure/certification can be used in lieu of education or experience, if applicable.
