Job summary

Ascension in Mequon, WI seeks a Registered Nurse for Inpatient Acute Rehab. You will coordinate care, manage care plans, and document in the EHR while collaborating with physicians, therapists, and social workers to ensure safe discharges.

The role requires WI RN licensure or multi-state/Compact, and BLS within 30 days. This full-time night-shift position supports a Catholic health system known for service and growth.

• RN licensure in Wisconsin (or multi-state/Compact) is required before hire.
• BLS Provider certification within 30 days of hire date is required.

Responsibilities

• Care Coordination: Acts as the central coordinator, ensuring seamless management and interdisciplinary collaboration for optimal patient outcomes.
• Care Plan Management: Implements, monitors, and adjusts the prescribed patient care plan based on findings and team input.
• Order Management: Interprets, documents, and executes physician and nursing orders including meds, tests, treatments, procedures.
• Clinical Observation, Documentation, and Communication: Assesss patient, documents findings in the EHR, and communicates changes to the team.
• Discharge Planning: Assesses discharge needs and arranges post-acute resources with patient, family, social work, and physicians.

• Care Coordination: Acts as the central coordinator, ensuring seamless management and interdisciplinary collaboration (physicians, specialists, therapists, social workers, ancillary staff) for optimal patient outcomes.
• Care Plan Management: Implements, continuously monitors, and dynamically adjusts the prescribed patient care plan based on clinical findings, patient progress, and team input, requiring critical thinking and timely decision-making.
• Order Management: Precisely interprets, documents, and executes all authorized physician and nursing orders, including medications, tests, treatments, and procedures.
• Clinical Observation, Documentation, and Communication: Maintains constant vigilance, systematically assesses the patient, meticulously documents findings and treatments in the EHR, and immediately communicates all significant changes to the physician and healthcare team.
• Discharge Planning: Proactively assesses discharge needs from admission, collaborating with the patient, family, social work, case management, and physicians to arrange post-acute resources and provide education for a safe transition of care.

What Minimum Requirements You’ll Need

Licensure / Certification / Registration:

• BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date required. American Heart Association or American Red Cross accepted.
• Registered Nurse credentialed from the Wisconsin Board of Nursing obtained prior to hire date or job transfer date required. Licensure from the Wisconsin Board of Nursing OR current home state license if considered multi-state/Compact State.

Education

• Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire.
