Job summary

Elara Caring is seeking a Scheduler to support Administrators in planning patient visits and coordinating care across clinical teams. You will manage schedules, ensure timely visits, and act as a liaison among CTMs, direct care staff, physicians, and facility personnel.

The role requires a high school diploma or GED, with preferred associates degree and at least 1 year scheduling experience in home care. Bilingual in Spanish and English is preferred; reporting to the Administrator or Branch

Qualifications

• High school diploma or GED required; Associates degree preferred.
• 1 year of practical scheduling experience in home care is preferred.
• Familiarity with national standards of care and licensure is helpful.

Responsibilities

• Schedule patient visits and manage direct care team member calendars.
• Coordinate with CTM, physicians, and facility staff to ensure timely care.
