Job summary

Hendry Regional Medical Center seeks a Patient Care Navigator to coordinate access to healthcare resources, scheduling primary care appointments, and managing referrals. The role serves as the central point of communication among patients, providers, and clinical staff to ensure timely and efficient care delivery.

You will coordinate internal and external referrals, obtain authorizations, schedule appointments, and track outcomes.

Qualifications

• High school diploma or equivalent required.
• Two years of outpatient clinic experience required.
• Working knowledge of referral and prior authorization processes, payer benefit structures, and medical terminology required.
• Experience in patient advocacy, case management, or care coordination preferred.
• Strong interpersonal and communication skills to interact with diverse patient populations and healthcare professionals.
• Basic knowledge of healthcare systems, insurance processes and community resources required.
• Ability to manage multiple tasks, maintain confidentiality, and demonstrate empathy and cultural sensitivity.

Responsibilities

• Gather, verify, and document patient demographics, financial and health information in hospital systems.
• Serve as the first escalation point for clinical support staff and front office on patient care/workflow issues.
• Monitor appointment availability and escalate risks to avoid access delays.
• Transcribe and interpret provider orders; document in the EMR.
• Coordinate internal and external referrals and authorize processes.
• Communicate with patients to schedule/confirm appointments and maintain continuity of care.
• Manage referrals to internal and external services and ensure timely completion.
