Qualifications

• 2+ years’ related work experience in a healthcare setting.
• Proficiency with Electronic Medical Record (Cerner preferred).
• Excellent interpersonal, customer service, verbal and written communication skills.
• Knowledge of Microsoft Office and proficient typing skills.
• Ability to manage and effectively follow up on completion of assigned duties.
• Demonstrated ability to take ownership of assigned duties and responsibilities.

Responsibilities

• Coordinates all approved off-site appointments with participating providers.
• Acts as liaison between Health Center and off-site providers.
• Ensures referrals are authorized and scheduled within set timeframes.
• Follows Patient Services workflow from start to finish.
• Updates knowledge of external partners’ requirements for off-site referrals.
