Job summary

Greenbrook is seeking a compassionate Case Manager to guide patients through their treatment journey in clinic settings across Northampton, Weymouth, Leominster, or Westborough, MA. You will coordinate care, manage schedules, support insurance and prior authorizations, and help patients overcome barriers to access and treatment.

Qualifications

• Bachelor's degree and at least one year of experience in patient care coordination or case management.
• Experience with EHR and CRM systems such as AdvancedMD and Salesforce.
• Familiarity with insurance benefits and prior authorizations is preferred.

Responsibilities

• Coordinate care from consultation through treatment completion to ensure smooth access.
• Schedule appointments, align provider calendars, and manage treatment timelines.
• Monitor patient progress and address barriers to timely care.
• Collaborate with Authorization Specialists and clinical teams to keep plans on track.
• Support prior authorization workflows and ensure coverage throughout treatment.
• Coordinate specialty pharmacy activities and treatment readiness requirements.
• Maintain accurate patient records in CRM and EHR systems.
• Respond to patient questions with empathy and urgency.
• Help deliver a high-quality patient experience while meeting compliance standards.
