Job summary

Greenbrook is seeking a compassionate Case Manager to serve as the primary contact for patients throughout their treatment journey. This clinic-based role coordinates care, schedules, supports insurance and prior authorizations, and helps patients overcome barriers to treatment.

You will collaborate with Authorization Specialists, providers, clinical teams, and revenue cycle partners to keep treatment plans on track, monitor progress, and maintain accurate records in CRM and EHR systems while

Qualifications

• Bachelor's degree and at least one year of experience in patient care coordination, healthcare operations, or a specialty care setting.
• Experience with EHR and CRM systems (AdvancedMD and Salesforce preferred).
• Knowledge of insurance benefits, prior authorizations, or healthcare scheduling is valued.

Responsibilities

• Guiding patients from consultation through treatment completion, ensuring a smooth and supportive experience.
• Coordinating appointments, provider schedules, and treatment timelines to maximize patient access and clinic efficiency.

Qualifications

Preferred

• Bachelor's degree and at least one year of experience in patient care coordination, case management, healthcare operations, or a specialty care setting.

Also Considered

• High School Diploma or Associate Degree with two or more years of relevant healthcare coordination or patient support experience.

Additional Experience

• Experience working with EHR and CRM systems (AdvancedMD and Salesforce preferred).


• Proficiency with Microsoft Office, including Outlook, Word, and Excel.


• Knowledge of insurance benefits, prior authorizations, or healthcare scheduling is highly valued.

Preferred Experience

Candidates with experience in any of the following areas are strongly encouraged to apply:

Selected candidate will be eligible to participate in the Company's employee benefit plans, which currently include: health insurance; dental insurance; vision insurance; optional supplemental medical insurance; health savings and flexible spending accounts; company-paid life insurance and additional voluntary life insurance; company-paid short term disability and long term disability insurance; participation in a 401(k) savings and investment plan with company contribution; and paid time off for non-exempt salaried employees is calculated as .625 per pay period worked for vacation time and .29166 per pay period for personal time. Full-time exempt employees will be eligible for Flex-PTO, which allows time off without accrual.
