Notch in Vermont is seeking a Primary Care Coordinator to improve patient health outcomes by coordinating care, managing resources, and collaborating with patients, providers, and community partners. The role focuses on medical, mental health, substance use, and social determinants of health, utilizing EMR tools and referrals to identify high-risk patients.

The position requires a Nursing background, RN licensure, and at least three years of clinical experience.

Competenze

• Bachelor’s Degree in Nursing or equivalent health-related field preferred.
• Three years’ experience in clinical setting preferred.
• Registered Nurse license required; BLS certification required or ability to obtain.

Mansioni

• Interact with patients and care team to provide care services.
• Serve as NOTCH liaison to 3rd party payers and manage medication authorizations.
• Perform medication reconciliation and track diagnostic imaging orders.
• Coordinate transitions of care and education for patients.
• Support disease management and social determinants of health.
• Maintain and update medical records and documentation as required.
• Collaborate with home health, facilities, hospitals, and community partners.
• Participate in quality improvement projects and staff training.
• Conduct home visits as needed and engage in community events.
