PVH Potomac Valley Hospital in Keyser, WV is seeking a skilled Registered Nurse to triage patient calls, assess needs, and coordinate referrals across clinics. You will educate patients on immediate care advice and preventative behaviors, while documenting interactions to maintain a comprehensive medical record.

The role requires at least 3 years of nursing experience, WV RN license or eNLC, BLS within 30 days, and prefers a BSN with care coordination experience.

Formación

• Current unencumbered RN license in WV or eNLC.
• BLS certification within 30 days of hire.
• Minimum 3 years of nursing experience.
• BSN preferred.
• Care coordination experience preferred.

Responsabilidades

• Triage callers’ needs using established protocols and document interactions.
• Provide callers with appropriate medical information using nursing background and available resources.
• Identify gaps in care and intervene when possible.
• Coordinate referrals and telehealth visits with providers.
• Assist patients in selecting physicians/facilities to optimize access and cost.
• Educate patients on treatment options and preventive care.
