Job summary

Northwell Health seeks a Care Management professional to coordinate services for patients and families, focusing on high-risk individuals and those with barriers to access.

You will work with the primary care team using motivational interviewing, manage panels to reduce missed follow-ups, and liaise with hospitals, specialists, and community resources to ensure seamless, patient-centered care.

Qualifications

• High School Diploma or equivalent required.
• 1-3 years of relevant experience, required.
• Bachelor's or Master's degree in relevant field strongly preferred.
• Health home or care management experience strongly preferred.
• NYS Driver's license strongly preferred.
• Bilingual Spanish preferred.

Responsibilities

• Utilizes patient-centered motivational interviewing to build rapport and help patients improve their health.
• Supports panel management to decrease lost to care and non-compliance; develops individualized care plans for high-risk patients.
• Acts as liaison between hospitals, primary care providers, specialists, and community resources for patient-centered care coordination.
• Identifies and tracks high-risk populations and others due for preventive or chronic care services.
• Identifies and tracks patients discharged from inpatient service or the emergency department.
• Uses team-based communication to close the loop on referrals and follow-ups; conducts outreach in clinics, homes, hospitals, and neighborhoods.
