The Patient Care Manager Assistant assists the Patient Care Manager in coordinating staff to ensure high-quality, efficient care for home health patients, in accordance with physician orders and regulations.

Responsibilities include:

• Handling communications with field staff, physicians, referral sources, and patients
• Monitoring incoming faxes and reporting information
• Coordinating communication and documentation with physician offices
• Following up on labs and communicating results
• Ensuring proper care coordination for hospital patients
• Managing software reports for care coordination and utilization management
• Assisting with payer authorizations
• Responding to health check calls or telemonitoring alerts
• Preparing patient lists and documents for case conferences
• Providing direct patient care as needed, within scope of practice
• Reviewing clinician documentation for accuracy and completeness

License requirements:

• Current CPR certification
• Current LPN licensure in the state of practice
• Valid driver’s license, vehicle insurance, and access to transportation
