Job Description

As a Licensed Vocational/Practical Nurse at Elara Caring, you will deliver high-quality care in patients' homes, following the Plan of Care and collaborating with the healthcare team. Your responsibilities include verifying care plans, documenting patient care, communicating significant findings, and ensuring patient safety.

Qualifications

• Graduate of an accredited nursing program (Certificate, Diploma, or Associate)
• Current State License as an LPN / LVN
• At least 1 year of clinical care experience
• Experience in hospice or home health preferred
• Willingness to travel 50%
• Ability to lift 50-100 lbs. and perform physical tasks
• Valid driver’s license, insurance, and reliable transportation
