Responsibilities

The Home Health Licensed Practical Nurse Patient Care Manager Assistant assists the Patient Care Manager in coordinating a team to ensure high-quality care to home health patients. This includes ensuring staff are skilled, informed, and committed to providing quality care according to physician's orders and regulations.

• Assist with phone calls from staff, physicians, referral sources, and patients; take messages and orders as needed
• Monitor incoming faxes and report information to relevant personnel
• Coordinate communication and send applicable documents to physician offices
• Monitor pending labs, facilitate follow-up, and communicate results to physicians and staff
• Monitor hospital status of patients to ensure proper care coordination
• Assist with managing software system reports for care coordination and utilization management
• Assist with payer source authorizations as needed
• Follow up on health check calls or tele-monitoring alerts, ensuring team communication
• Prepare patient lists and documents for weekly case conferences
• Provide direct patient care as necessary, following scope of practice and physician orders
• Assist in clinician documentation review and audits to ensure accuracy and completeness

Education and Experience

License Requirements:

• Current CPR certification
• Current LPN licensure in the state of practice
• Valid driver's license, vehicle insurance, and access to reliable transportation
