American Medical Staffing is seeking a LPN / LVN for a travel job in Kansas City, Missouri. The ideal candidate will have prior experience in home health and be familiar with HCHB. The position offers guaranteed weekly hours, primarily weekday day shifts for 40 hours.

Responsibilities

• Conduct comprehensive assessments and develop individualized care plans in collaboration with the interdisciplinary team.
• Provide skilled nursing care to patients in their homes.
• Educate patients and families on disease management, medication, and health topics to promote self-care.
• Monitor patient progress and adjust care plans as needed.
• Offer emotional support and counseling to patients and families coping with chronic or end-of-life conditions.
