Job summary

CareRing Health in Lancaster, PA seeks a Patient Care Manager to supervise home care services and ensure compassionate, high‑quality care for patients.

The role focuses on assessing eligibility, coordinating Direct Care Workers and patients, team leadership, and meeting performance goals while maintaining compliance and a strong work‑life balance.

Qualifications

• Experience in home care, nursing or other medical fields.
• Ability to build rapport with patients.
• Two or more years of healthcare leadership preferred.
• Bilingual preferred.

Responsibilities

• Conduct home assessments to determine consumer eligibility for Home Care services.
• Communicate with Direct Care Workers and patients about company updates.
• Supervise, observe and monitor contact visits.
• Advise, guide, and train household members in managing household affairs and care.
• Complete routine updates on the well-being of patients and direct care workers.
• Lead and supervise a team of Care Team Associates (CTA).
• Meet operational and growth goals, including KPIs.
• Ensure quality of services delivered meets Company standards.
• Foster an engaged workforce.
• Assist CTAs with day-to-day duties, onboarding, staffing patient cases and on-site visits as needed.

Responsibilities

• Conduct home assessments to determine consumer eligibility for Home Care services


• Effectively communicate with Direct Care Workers and patients about company updates regarding their services.


• To supervise, observe and monitor contact visits.


• To advise, guide, and train household members in managing household affairs', practical tasks, and other means of providing for care.


• Complete routine updates on the well-being of patients and direct care workers.


• Lead and supervise a team of Care Team Associates (CTA)


• Ensure that patients receive personal care services based on needs and what matters most to the patient


• Meet operational and growth goals, including key performance metrics


• Ensure quality of services delivered meets Company standards


• Foster an engaged workforce


• Ensure compliance with the law and Company policies


• Assist CTAs with their day‑to‑day duties, including screening new patients and caregivers, on‑boarding new patients and caregivers, staffing patient cases with caregivers and ensuring patient cases are started promptly, communicating with patients, caregivers, and patient families to ensure satisfaction and quality service delivery, and assist with on‑site visits as needed

Qualifications

• Previous experience in home care, nursing or other medical fields


• Ability to build rapport with patients


• Associate’s degree or Bachelor’s degree required


• Two or more years of healthcare leadership preferred


• Bilingual preferred


• Passion and dedication to help those in need
