Job summary

UCLA Health in Pasadena seeks an onsite or remote-capable professional to provide direct support to physicians and patients in assigned primary and specialty care practices. Reporting to the Manager, you’ll act as a liaison, help patients access services, coordinate with referring physicians, and guide follow-up via telephone and Epic/Care Connect.

• Ability to provide coverage for Community Clinics as assigned.

• Working knowledge of UC system services and programs.
• Knowledge of hospital computer systems and related software for admin work.
• Familiarity with authorization processes & managed care programs.

Responsibilities

• Reporting directly to the Manager, providing direct support to the physicians and patients for assigned primary and specialty care physicians.
• Acting as a liaison for the patients and physicians in the practice, assisting patients in accessing service within the health system, facilitating communication with referring physicians, referring patients to resources and providing follow-up through telephone and EPIC/Care Connect.
