Shawnee Health, a leading healthcare provider in Southern Illinois, is seeking a Patient Care Coordinator I to join our Shawnee Health office in Carbondale. This role reports to the Patient Care Coordinator Supervisor and focuses on coordinating referrals, pre-authorizations, and patient communications within the EMR system.

The ideal candidate has strong computer skills, independent problem-solving abilities, and excellent written and verbal communication.

Qualifications

• Must be competent in the use of computers.
• Must be capable of independent problem solving.
• Must be able to work with little direct supervision.
• Must have excellent written and verbal communication skills.

Responsabilités

• Schedule medical referrals, diagnostic tests and treatments and notify patients and document in EMR.
• Review and select medical records for referrals and tests to include with orders.
• Complete pre-authorizations with insurance related to referrals/tests.
• Follow up on referrals/tests to ensure completion and receipt of reports.
• Notify provider of incomplete referrals/tests.
• Work on related EMR tasks as assigned.
• Assist social service team with patient needs including financial, transportation, interpretation, etc.
• Complete prior authorization processes when required.

Shawnee Health is seeking aPatient Care Coordinator Ifor our Shawnee Health office. This position reports directly to the Patient Care Coordinator Supervisor.

Responsibilities

• Schedule medical referrals, diagnostic tests and treatments ordered by provider and assure that the patient has been notified, understands directions, and completes all documentation in the patient’s Electronic Medical Records.
• Reviews and selects medical records pertinent to referrals, diagnostic tests and treatments to be sent with order to provider or facility
• Completes the pre-authorizations process with insurance companies as it relates to the referral and/or diagnostic test scheduled.
• Follows up on referrals and diagnostic tests ordered to ensure completion and the receipt of all reports including consult notes and test results.
• Notify provider of referrals or tests that have not been completed by patient.
• Works to complete EHR task as assigned
• Works directly with the social service team assisting with the identification of patients with special needs including but not limited to: financial assistance, transportation, pharmaceutical assistance, interpretation, or special accommodations.
• Completes prior authorization process for referrals and diagnostic testing when required.
• Assist Patient Care Coordinator Supervisor with training new employees.
• Monitors access issues with referrals and provides consistent feedback to Patient Care Coordinator Supervisor

Starting salary – $16.00
