Job summary

Lorian Health in San Jose is seeking a Patient Care Coordinator to support intake of referrals and coordinate clinical field staff schedules for admissions, discharges, resumption of cares, and routine follow-ups. You will also assist in answering the phone and maintaining service levels.

The role requires strong communication, Microsoft Office proficiency, and HIPAA privacy adherence to protect patient information and ensure accurate documentation in the HomeCare HomeBase system.

Qualifications

• Working knowledge of Medical terminology.
• Proficient with Microsoft Office and EMR (HCHB preferred).
• Strong customer-service and phone skills.
• Ability to organize and prioritize workload.
• Maintains HIPAA privacy standards.

Responsibilities

• Handles intake of new referrals and maintains all clinical staff schedules.
• Acts as liaison between Lorian Health and referral sources.
• Coordinates admissions, discharges, resumption of cares, recertifications, and routine follow ups.
• Ensures patient information is accurately entered into HomeCare HomeBase to meet Medicare requirements.
• Collaborates with sales and clinical management on new referrals.
