Job summary

Ennoble Care in Gulf Breeze, FL is seeking a full-time Patient Care Coordinator to join our in‑office clinical care team. You will support patient-centered care coordination, documentation in the EMR, and communication between providers and internal care teams.

This role requires strong organization, proactive communication, and attention to detail, with standard daytime hours, no weekends or on‑call, and seven paid company holidays.

Qualifications

  • Legally authorized to work in the United States.
  • Active, unrestricted clinical certification or licensure (PCT, CNA, MA/CMA, LPN, RN, or comparable).
  • Minimum 1 year of clinical and administrative or care coordination experience.
  • Strong written and verbal communication via Microsoft Teams, email, phone, and internal systems.
  • Excellent organizational, time-management, and detail orientation.
  • Proficient in healthcare terminology and care coordination principles.
  • Proficient with Microsoft 365 (Outlook, Word, Excel).
  • Able to work full-time, Mon-Fri, 7:00-3:30 CST, Gulf Breeze, FL; no weekends/on-call; 7 holidays.
  • Ability to pass a background check.

Responsibilities

Ennoble Care is seeking a full‑time Patient Care Coordinator to join our team in an office‑based setting in Gulf Breeze, Florida. This role is an integral, in‑office member of our clinical care team, working alongside fellow Patient Care Coordinators and collaborating with Ennoble Care providers within a family‑practice‑style model of care. While not patient‑facing, the position plays a critical role in supporting patient‑centered care coordination, documentation, and communication, helping ensure continuity of care and timely follow‑up across care settings. Key Responsibilities
  • Coordinate patient‑centered care activities using Ennoble Care’s electronic medical record (EMR) system
  • Support Ennoble Care providers and clinical teams through accurate documentation, follow‑up tracking, and care coordination
  • Serve as a key point of communication between providers and internal care teams regarding clinical updates, patient needs, and care coordination priorities
  • Communicate patient needs, changes in condition, and urgent concerns to the clinical team in a timely and appropriate manner
  • Assist with care transitions, referrals, and coordination with external partners, as needed
  • Collaborate closely with fellow Patient Care Coordinators and internal care team members within an office‑based environment
  • Maintain accurate, timely documentation in accordance with established workflows, documentation standards, and escalation protocols to support timely decision‑making and efficient clinical operations
  • Contribute positively to team collaboration, workflow efficiency, and day‑to‑day office operations
Requirements
  • Legally authorized to work in the United States of America
  • Active, unrestricted clinical certification or licensure, such as:
    • Patient Care Technician (PCT)
    • Certified Nursing Assistant (CNA)
    • Medical Assistant (MA) / Certified Medical Assistant (CMA)
    • Licensed Practical Nurse (LPN)
    • Registered Nurse (RN)
    • Or comparable credential
  • Minimum 1 year of experience in a clinical and administrative or patient care coordination role within a healthcare setting
  • Comfortable communicating proactively and professionally throughout the day via Microsoft Teams, email, phone, and internal systems
  • Cultivate and maintain positive, approachable working relationships while demonstrating a consistent, dependable presence
  • Well‑developed organizational, time‑management, and attention‑to‑detail skills, paired with an optimistic approach and initiative‑driven mindset
  • Strong working knowledge of healthcare terminology and care coordination principles
  • Proficiency with Microsoft 365, including Outlook, Word, and Excel
  • Ability to work full‑time, Monday through Friday, 7:00 a.m. to 3:30 p.m. CST, in an office‑based setting in the Gulf Breeze, Florida area
    • No weekends
    • No on‑call
    • Daytime hours
    • Seven (7) company‑paid holidays annually
  • Ability to pass a background check
