You will work on-site, Monday through Friday, 8:30 a.m.–5:00 p.m., with full-time benefits and opportunities to grow within a compassionate team that values patient-centered care.

Qualifications

• Comfortable speaking on the phone for large portions of the day.
• Compassionate and empathetic towards patients, consistently demonstrating exceptional customer service.
• Take accurate notes to document each task in a timely manner.
• Multitask between different patients and workstreams while remaining organized and efficient.

Responsibilities

• Complete individualized patient care plans and perform care management and coordination services using Ennoble Care’s electronic medical record system.
• Frequent contact with patients to provide care coordination, support, and manage compliance with care management programs to increase positive outcomes.
• Document all client communications (verbal or written) accurately.
• Communicate with Primary Care Clinician or designee regarding patient emergent needs and/or life‑threatening episodes, ensuring comprehensive care plans are complete and accurate.
• Keep Team Supervisor informed of all issues pertinent to the care plan process and any known or perceived issues.

Patient Care Coordinator

Full‑time, experienced Patient Care Coordinator role based in Reston, Virginia. You will align with our motto, "To Care Is An Honor", and ensure Ennoble Care delivers high quality care services. You will work with clinicians, staff, and patients to reach healthcare goals and maintain open communication, managing individual care providers and supporting caregivers and families.

Responsibilities

• Complete individualized patient care plans and perform care management and coordination services using Ennoble Care’s electronic medical record system.
• Frequent contact with patients to provide care coordination, support, and manage compliance with care management programs to increase positive outcomes.
• Document all client communications (verbal or written) accurately.
• Communicate with Primary Care Clinician or designee regarding patient emergent needs and/or life‑threatening episodes, ensuring comprehensive care plans are complete and accurate.
• Keep Team Supervisor informed of all issues pertinent to the care plan process and any known or perceived issues.
• Demonstrate ability to work with various cross‑organizational areas to meet the needs of Ennoble Care’s patients, their family members, and partner facilities.
• Become skilled at using technology including secure email, telephone system, electronic medical records, etc.
• Adhere to documentation protocols and best practices for daily work logs, escalation of client issues, and internal communications.
• Provide excellent customer service, evidenced by positive feedback from customers and patients.
• Support all members of the department in a productive and constructive manner.

Equipment Operation

• Utilization of a computer, telephone, copy machine, and other office equipment as necessary.

Qualifications

• Proficient using a computer, including Outlook and other Microsoft Office programs.
• Knowledge of basic healthcare terms, conditions, roles, and basic care principles.
• Pass a drug screen and background check; possess a positive attitude, adapt positively to change, be a team player, and be willing to learn new skills on a continuous basis.

Pay starts at $17.00 – $18.00 per hour.
