Job summary

Guidehouse is seeking a Patient Care Coordinator to support efficient care delivery. You will coordinate patient activities and serve as a liaison among patients, clinicians, and administrative teams.

Responsibilities include scheduling, referrals, follow-ups, intake processes, documentation, and record maintenance. Travel up to 10% and must obtain a Public Trust. Associate degree required; 1+ year healthcare coordination experience preferred.

Qualifications

• Associate degree required; bachelor’s preferred.
• Minimum 1 year in patient coordination, healthcare administration, or clinical support.
• Strong communication and patient-centered approach; HIPAA knowledge a must.
• Familiarity with healthcare workflows and care management processes.
• Ability to handle sensitive information with discretion.

Responsibilities

• Coordinate patient-related activities to support efficient care delivery.
• Serve as liaison between patients, clinical staff, and administrative teams.
• Assist with scheduling, referrals, follow-ups, and care coordination.
• Support patient intake, documentation, and record maintenance.
• Ensure patients receive timely information and support throughout care.
• Track patient interactions using established systems; maintain privacy and compliance.
