Job Summary:
The Patient Care and Staffing Director assumes the responsibility of coordinating of care and staffing functions in accordance with state, federal, and local regulations.
Reports to: Clinical Director/Clinical Manager
Job Qualifications:
- Education: Current Texas license as a Licensed Vocational Nurse from the Board of Nurse Examiners.
- Experience: One year of experience as a Licensed Vocational Nurse in Home Health preferred.
- Skills: Nursing Skills as defined by accepted standards of practice; good people skills. Excellent verbal, written communication, and people skills required. Knowledge of office machines, computer skills required; excellent interpersonal and organizational skills; staffing skills. Strong Typing Skills Required.
- Transportation: Reliable transportation and valid auto liability insurance.
Essential Functions:
- Responsible for the overall management of patient care to include scheduling, discipline utilization, orders, authorizations, hospitalization tracking, and coordination of care with MDs. Responsible for supplying the needs of all patients of the Agency.
- Assess, plan, and coordinate changes in patient care services.
- Assist in the development of the plan of care and or coordination (POC/CC) through obtaining specific diagnosis, physician orders, and reasons for admission. Responsible for contacting field RN Case Managers and/or Supervising Therapist with required POC/CC information under the supervision of the Clinical Manager/Director.
- Initiates verbal physicians' orders as needed and communicates changes with field staff.
- Knowledgeable about all local, state, and federal regulations.
- Assigns and verifies that all nurses and other disciplines comply with documentation (timely submission and QA) and that the daily schedule is being followed.
- Provides staffing feedback and reports any deficiencies to the Clinical Manager/Director.
- Maintains and updates patient information to enhance the highest-quality patient care.
- Consults with all treatment team members and Clinical Manager/Director regularly to coordinate quality, goal-oriented home health care.
- Serves as liaison between patients, caregiver(s), physicians, physician’s office staff, Agency administrative staff, and contracted services.
- Assists the Clinical Manager/Director with certification and recertification process/determination by managing and leading weekly case conferences.
- Establishes, monitors, and evaluates productivity standards by conducting chart audits for the completion of all appropriate documentation.
- Monitor hospitalizations and obtain necessary orders and clinicals for resumption of care. Coordinates re-hospitalizations, transfer of care, and discharges with the following providers.
- Promotes Agency philosophy and administrative policies to ensure quality of care.
- Utilizes the EMR, VigeoDash, and all available technology platforms to ensure proper clinical utilization and Insurance requirements/authorization Management.
- Enhances professional growth and development through participation in education programs, staff in-services, and workshops.
- Assist with performing patient care visits per agency needs.
- Perform other functions as assigned.
Environmental and Working Conditions:
- Works in a routine office environment. The noise level may be moderately high.
- The ability to work a flexible schedule and extended hours.
- The ability to travel locally with some exposure to inclement weather.
Physical and Mental Effort:
- Prolonged sitting and some standing are required.
- Occasional need to lift, pull, carry, and push items weighing up to 50 pounds.
- Frequent need to stoop, kneel, and reach while accessing files.
- Requires working under some stressful conditions to meet deadlines and employer/employee needs. Requires hand-eye coordination and manual dexterity.
- Requires excellent critical thinking skills.
