What is a Patient Care Coordinator?
- An entry-level office role responsible for maintaining the daily operations of the clinic.
- Excellent customer service skills are essential.
- Multitasks across multiple computer programs daily and learns new skills quickly.
A day in the life of a Patient Care Coordinator:
- Greet everyone entering the clinic warmly.
- Schedule new referrals received via fax or phone from patients and physician offices.
- Verify insurance coverage for patients.
- Collect patient payments.
- Maintain an organized front office workspace.
- Perform other duties as assigned.

Position Summary:
The Patient Care Coordinator - I (PCC-I) supports clinic growth through excellent practice management and patient intake. Works with the Clinic Director to ensure efficient procedures, supporting the mission, vision, and values of Upstream Rehabilitation.
Responsibilities:
- Collect payments at the time of service.
- Convert referrals into evaluations.
- Schedule patient visits.
- Provide excellent customer service.
- Create an inviting clinic atmosphere.
- Make welcome calls and monitor patient arrival rates.
- Manage schedules, documentation, deposits, caseloads, and inventory.
- Attend training sessions and complete compliance training as required.
Qualifications:
- High School Diploma or equivalent.
- Strong communication skills to relate well with leadership and team members.
- Ability to multitask, organize, meet deadlines, and work independently.
- Effective time management and presentation skills.
Physical Requirements:
- Primarily indoor environment, sedentary work with some movement.
- Full-time hours, Monday through Friday, with occasional evening or weekend events.
- Ability to sit, communicate, operate office equipment, and view screens constantly.
- Ability to occasionally lift or carry up to 20 lbs.
