Humana Inc. is seeking a Patient Care Coordinator to develop and maintain relationships with hospitals, SNFs, and discharge planning services, assisting in determining patient eligibility and coordinating resources.

You will conduct facility visits at the physicians' request to support the clinical team and participate in weekly business development meetings. The role requires an RN/LPN/LVN license, a BSN preferred, and about 3 years of clinical experience.

Qualifications

  • RN/LPN/LVN license required.
  • BSN preferred.
  • 3 years clinical experience preferred.
  • Strong knowledge of governmental regulations and Medicare eligibility requirements.
  • Excellent customer service, communication and selling skills.
  • Willingness to travel within assigned territory and to sales meetings.
  • TB screening program participation if selected.

Primarily conduct facility visits at the physicians' request to assist program clinical team in determining eligibility. Ensure effective communication and collaboration with program staff and other field sales resources via weekly meetings. Actively participate in weekly program business development meetings. Assist program in processing of physician orders as directed. You will report to a Market Executive of Sales Use your skills to make an impact

Required/Preferred Experience/Skills:

  • RN/LPN/LVN license is required.
  • Bachelor's of Science in Nursing preferred.
  • 3 years clinical experience preferred.
  • Must have strong knowledge of governmental regulations, Medicare eligibility requirements, comprehensive understanding of potential care plan needs for the patient and coordination of necessary resources.
  • Excellent customer service, account development capabilities, organization, time management, problem-solving, communication and selling skills.
  • Travel within an assigned territory and to sales meetings as required.
  • This role is considered patient facing and is part of Human’s Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB.

Scheduled Weekly Hours 40 Pay Range The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc. $59,300 - $80,900 per year
