Job summary

CenterWell Home Health seeks a Patient Care Coordinator to develop and maintain contact with discharge planning services and manage updates on our services within the market. You will conduct facility visits at physicians' request to help determine client eligibility and coordinate care with the clinical team.

You will collaborate with program staff and field sales resources, participate in weekly meetings, and assist with processing physician orders.

Qualifications

  • RN or LPN or LVN license is required.
  • BSN preferred.
  • 3 years clinical experience is preferred.
  • Knowledge of governmental regulations and Medicare eligibility requirements; care coordination for patient needs.

Responsibilities

  • Develop/maintain contact with key hospital, skilled nursing, and discharge planning services to provide updates on CenterWell Home Health offerings.
  • Conduct facility visits at physicians' request to assist program clinical team in determining eligibility.
  • Ensure effective communication and collaboration with program staff and field sales resources; participate in weekly program business development meetings.
  • Assist program in processing of physician orders as directed.
  • Report to a Market Executive of Sales.

Skills

RN/LPN/LVN license required

Education

  • This role is considered patient facing and is part of Human's Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB.

Pay Range

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc. $53,400 - $72,800 per year
