Job summary

CenterWell Home Health is hiring a Patient Care Coordinator in the United States. The role focuses on developing and maintaining relationships with hospitals and facilities, assisting with discharge planning, and coordinating care resources.

You will visit facilities to assess eligibility and collaborate with program staff in weekly meetings. A nursing license is required, with BSN preferred, and travel within the territory is expected.

Qualifications

• RN/LPN/LVN license is required.
• Bachelor's of Science in Nursing preferred.
• 3 years clinical experience preferred.
• Must have strong knowledge of governmental regulations, Medicare eligibility requirements, comprehensive understanding of potential care plan needs for the patient and coordination of necessary resources.
• Excellent customer service, account development capabilities, organization, time management, problem-solving, communication and selling skills.
• Travel within an assigned territory and to sales meetings as required.
• This role is considered patient facing and is part of Human’s TB screening program; TB screening is required if selected.

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.

$59,300 - $80,900 per year
