Job summary

Humana Inc. CenterWell Home Health is seeking a nurse liaison to develop and maintain relationships with hospitals and long-term care facilities. The role involves coordinating discharge planning services and monitoring eligibility while collaborating with the clinical team.

You will conduct facility visits, participate in weekly meetings, and assist physicians with orders. A valid RN/LPN license is required, BSN preferred, and travel within the territory is expected.

Qualifications

  • RN or LPN license required.
  • Bachelor's of Science in Nursing preferred.
  • 3 years clinical experience preferred.
  • Knowledge of governmental regulations and Medicare eligibility.
  • Excellent customer service and selling skills.
  • Travel within territory and to meetings.
  • Role is patient-facing and part of TB screening program.

Job description

Responsibilities
  • Develop/maintain contact with key hospital, skilled nursing, assisted living facility discharge planning services and/or management to provide ongoing updates on Company's services available in a market.
  • Conduct facility visits at the physicians' request to assist program clinical team in determining eligibility.
  • Ensure effective communication and collaboration with program staff and other field sales resources via weekly meetings.
  • Actively participate in weekly program business development meetings.
  • Assist program in processing of physician orders as directed.
Required/Preferred Experience and Skills
  • RN or LPN or LVN license is required.
  • Bachelor's of Science in Nursing is preferred.
  • 3 years clinical experience is preferred.
  • Must have knowledge of governmental regulations, Medicare eligibility requirements, comprehensive understanding of potential care plan needs for the patient and coordination of necessary resources.
  • Excellent customer service, account development capabilities, organization, time management, problem-solving, communication and selling skills.
  • Travel within an assigned territory and to sales meetings.
  • This role is considered patient facing and is part of Human's Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB.

Scheduled Weekly Hours 40

Pay Range The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc. $53,400 - $72,800 per year
