Job summary

CenterWell Home Health is seeking a Patient Care Coordinator in Iowa. You will work with the local sales team, coordinating patient care from hospitals and skilled nursing facilities, and meeting patients at their bedside to discuss home health services and what to expect at home.

The role requires an active RN, LPN, or LVN license in Iowa. Travel within the territory is required, and you will support the start of care by providing resources and coordinating with operations.

Qualifications

• Current RN, LPN or LVN license in Iowa is required.
• Three years clinical experience preferred.
• Knowledge of government regulations and Medicare eligibility is beneficial.
• Travel within assigned territory is required.
• TB screening will be required and participation is mandatory.

As a Patient Care Coordinator, You Will

• Support operations teams by ensuring they have the resources, information, and coordination needed to achieve a successful start of care.
• Assist with sales calls as needed to support Account Executive in addressing client needs and advancing business objectives.

Required/Preferred Experience/Skills

• LPN, LVN or RN license is required.
• Knowledge of governmental regulations, Medicare eligibility requirements, comprehensive understanding of potential care plan needs for the patient and coordination of necessary resources.
• Three Years Clinical Experience Is Preferred.
• Travel within assigned geographical territory is required.
• This role is considered patient facing and is part of Human's Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB.

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.
