Job summary

Advanced Infusion Care (AIC), a division of AIS Healthcare, is seeking a Clinical Support Coordinator to join our Garden Grove team onsite. The role focuses on scheduling nursing services, coordinating referrals with Intake, Pharmacy, Nursing, and Home Health partners, and providing essential administrative support to ensure efficient operations.

You will serve as a key contact for patients and clinicians, maintain visit logs, and help deliver timely, accurate care while protecting patient

Qualifications

• High School Diploma or GED required.
• 1 year of clinical support, admin, scheduling, or related experience.
• Strong customer service and communication skills.
• Excellent organizational abilities and attention to detail.
• Ability to manage multiple tasks and deadlines in a fast-paced setting.
• Proficiency with Office apps and healthcare software.
• HIPAA privacy awareness helpful.

Responsibilities

• Coordinate scheduling for new patient referrals and nursing visits.
• Collaborate with Intake, Pharmacy, and Nursing teams for timely scheduling.
• Verify patient readiness and confirm visit dates.
• Inform patients about visits and status updates.
• Maintain Clinician Visit Logs (CVLs).
• Monitor emails and faxes; route information as needed.
• Answer calls and direct to proper department.
• Process messages accurately and promptly.
• Provide information about services, contact details, and resources.
• Assist staff with administrative tasks and paperwork.
• Protect patient privacy per HIPAA.
• Support team goals and knowledge sharing.
• Perform other duties as assigned.

Position Summary

The Patient Care Coordinator is responsible for creating patient schedules for nursing services and providing administrative support to ensure efficient day-to-day operations.

This role works closely with Intake, Pharmacy, Nursing, and Home Health partners to coordinate new referrals, ongoing patient visits, and clinician schedules. The Clinical Support Coordinator serves as a key point of contact for patients, clinicians, and internal teams to ensure services are delivered accurately and efficiently.

Key Responsibilities

• Coordinate scheduling for new patient referrals and ongoing nursing visits
• Work closely with Intake, Pharmacy, and Nursing teams to ensure timely and efficient scheduling
• Verify patients are ready to be scheduled for services
• Determine nursing availability and confirm visit dates with patientsKeep patients informed regarding upcoming visits and order status updates
• Coordinate patient visits with home health agencies
• Create and maintain Clinician Visit Logs (CVLs)
• Monitor incoming emails and faxes throughout the day and route information appropriately
• Answer incoming phone calls and direct callers to the appropriate department or team member
• Take and distribute messages accurately and promptly
• Provide information regarding company services, contact information, and resources
• Assist staff with administrative support as requested
• Complete assigned paperwork and documentation accurately and within established timeframes
• Protect patient privacy and confidentiality in accordance with HIPAA requirements
• Share knowledge and collaborate with team members to support departmental goals
• Perform other duties as assigned

Minimum Qualifications

• High School Diploma or GED
• Minimum of 1 year of clinical support, administrative assistant, scheduling, customer service, or related experience
• Strong customer service and communication skills
• Exceptional organizational skills and attention to detail
• Ability to manage multiple tasks and priorities simultaneously
• Ability to meet deadlines while maintaining accuracy in a fast-paced environment
• Proficiency with Microsoft Outlook, Word, Excel, and email applications
• Experience using standard office equipment, including computers, scanners, telephones, fax machines, and headsets
• Ability to learn and navigate healthcare software systems quickly

Preferred Qualifications

• Experience managing schedules and appointments
• Healthcare, pharmacy, infusion, or clinical office experience
• Experience supporting nursing or healthcare operations
• Experience coordinating services between multiple departments
