Case management is a process that efficiently and effectively aligns patient needs/issues with resources to meet quality, clinical and cost outcomes. Education And CredentialsCurrent New York State registration as a Registered Professional Nurse required. Bachelor of Science degree in Nursing required within five (5) years of hire to the position. Certified Case Manager (CCM) preferred. Employees in the job title prior to 3/29/10 are grandfathered and not required to obtain their BSN. Basic Life Support (BLS) required upon hire. Current NYS registration as a Registered Professional Nurse/RN Permit and American Heart Association-Basic Life Support (BLS) required upon hire and must be maintained thereafter. PRI certification required and will be completed during orientation period and maintained thereafter for new staff. Current staff will have until 03/31/2026 to obtain certification. Experience2 years of acute care Nursing Experience required. 2 years of experience in any combination of case management, home care and utilization review preferred. Knowledge of and experience with Interqual Criteria set and software systems preferred. Working knowledge of Microsoft Office Suite and Outlook required. Use of computer applications related to health care industry required.Working ConditionsEssential:
