Job Overview
Full‑time Patient Care Coordinator (PCC) at Ancora Home Health and Hospice. The role supports patients and referral sources by managing intake, insurance verification, authorizations, and information while maintaining strict privacy and confidentiality.
Responsibilities
- Serve as the primary point of contact for referral sources and patients, receiving and processing phone calls.
- Collect essential patient and insurance information, verify coverage and authorizations, and submit required documentation.
- Maintain accurate, up‑to‑date patient records in compliance with privacy regulations.
- Coordinate with internal teams to ensure timely and appropriate care planning.
- Manage multiple tasks in a fast‑paced environment while demonstrating strong attention to detail.
Qualifications
- 1–2 years of medical office or related experience.
- Service‑oriented mindset with a compassionate, caring attitude.
- Excellent verbal and written communication skills.
- Strong time‑management and multitasking abilities.
- Valid driver’s license, current auto insurance, and reliable transportation.
- Clean criminal background check.
- Current CPR certification and negative TB test.
