AdventHealth in Daytona Beach, FL is seeking a part-time Patient Care Coordinator to support care coordination, patient transportation, and documentation under RN or Social Work supervision. The role emphasizes strong communication with patients, families, physicians, and care managers.

The ideal candidate has a high school diploma, solid computer skills, and some office experience; healthcare experience is preferred.

Qualifications

• Must have high school diploma or equivalent.
• Experience with patient coordination preferred.
• Strong computer literacy including MS Office.
• Excellent communication and interpersonal skills.
• Knowledge of medical terminology is helpful.

Responsibilities

• Promotes excellent customer services through interactions with patients, physicians, care managers, and the healthcare team.
• Collaborates with the Care Managers for patient care coordination needs.
• Coordinates patient care and services arrangements under the direction of a RN or SW Care Manager and in collaboration with the patient/family.
• Provides and documents the delivery of regulatory letters to patients or their representatives in accordance with Federal and State regulations.
• Arranges patient transportation and follow up appointments as requested.
